Calligraphy Workshop

I acknowledge that I have voluntarily consented to partake in the Sister's Calligraphy Workshop Registration offered by ISNA Canada taking place on February 5, 2025. I hereby release and agree to hold ISNA Canada and their partners harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or that may otherwise arise in any way in connection with any services received from ISNA Canada. I understand that this release discharges ISNA Canada from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from ISNA Canada. This liability waiver and release extends to the Community Center together with all owners, partners, and employees. All reasonable precautions will be taken to ensure safety and prevent personal harm or injury (including contracting COVID-19 or any other illnesses). However, incidents related to activities may occur and cause injury, damage, or loss of property through no fault of ISNA Canada. I therefore wholly absolve ISNA Canada and all of their partners, agents, principals, organizers, parent and subsidiary agencies and organizations from all responsibility of personal injury, loss or damage in any form and by any means. I am aware that walking, hiking, etc. have the usual dangers and risks inherent in outdoor activities, some of which are: 1) sunburn, windburn; due to varying weather conditions 2) personal injury such as cuts and scrapes, broken bones, pulled muscles, tendons, and sprains; due to natural hazards and terrain variations en route. I accept all the inherent risks of the above activities and the possibility of personal injury, death, property damage or loss resulting therefrom. I hereby grant ISNA Canada the right to reproduce, use, exhibit, display, broadcast, distribute and create derivative works of the photographed images of me and my children, taken during this event, for use in connection with the activities of ISNA Canada or for promoting, publicizing or explaining ISNA Canada or their activities. This grant includes, without limitation, the right to publish such images on ISNA Canada websites, in our reports and publications and in PR/promotional materials, such as marketing admissions publications, advertisements, fund-raising materials, and any other ISNA Canada-related publication. These images may appear in any of the wide variety of formats and media now available to ISNA Canada and that may be available in the future, including but not limited to print, broadcast, video, and electronic/online media. I have read, understood, and agreed with this waiver statement. Signing and submitting this form acknowledges the agreement without reservation.
